Best Practices in Efficient Medical Care Coordination -

Event: 
Dr. Christine Schaeffer-Pettigrew, Medical Director of the Northwestern Follow Up Clinic, presents a cost-effective, thoughtful model of primary care that has successfully prevented rehospitalizations and empowered hundreds of low-income, medically complex patients to improve their health and livelihood.

This innovative, transitional model can be applied across the spectrum of health systems nationwide as the healthcare landscape shifts to an emphasis on coordinated, performance-based care.  Attendees who are interested in best practices in efficient medical care coordination will gain new insights regarding transitions of care from the hospital to the primary care provider’s office for some of our most vulnerable patient populations.

Speaker:
Christine Schaeffer-Pettigrew earned her doctorate at the University of Chicago Pritzker School of Medicine in Chicago. She remained at the University of Chicago for her post graduate education completing her residency in internal medicine in 2000. 

She has worked as a general internist and clinical educator at John Stroger Hospital of Cook County from 2001-2007. She then worked as an assistant program director for the internal medicine residency at Mercy Hospital of Chicago with a clinical practice in the Mercy Family Health Clinic, another FQHC, before coming to Northwestern as a co-founder of the Northwestern Follow-up Clinic. She has a long standing commitment to improving health care and health care access for the underserved.

1. Valet Parking (available only at Prentice Women's Hospital)
Twenty-four hour valet parking services are available through the Prentice driveway on Superior Street.

Valet rates are:
$23 for less than seven hours
$33 for seven to 24 hours
Please note: Parking rates are subject to change.

2. Self-Parking
Patients and visitors can park at a discounted rate in the structures at 222 E. Huron Street and 321 E. Erie Street. 
Validation is available in our hospitals and physician’s offices.

Parking rates are:
$10 for less than seven hours
$24 for seven to 24 hours
